 This document provides details of my privacy policy to comply with UK General Data Protection Regulation (GDPR) requirements and applies to you, the user of this website, and Charlotte Medlicott, the owner and provider of this website. Charlotte Medlicott is a data controller and processor, and takes the privacy of your information very seriously. To comply with the General Data Protection Regulation I am registered with the Information Commissioner (Registration Number C1203831).

The type of personal information I collect

I collect and process the following personal information which is provided by you from either the online form which is accessed through www.charlottemedlicott.co.uk/com, or from the client consent and intake forms which are completed on paper when we begin counselling: full name, date of birth, telephone number, email address, home address, relationship status, medication details, doctor’s name and address, next of kin name and telephone number. I also keep brief paper based notes of our therapy sessions

How I collect Data and why I have it

The personal information I process is provided to me directly by you for the sole purpose of providing counselling. I do not share this information with any third parties. By completing the online information request form, you are giving me consent to store your data. You are able to remove your consent at any time and can do this by contacting Charlotte Medlicott at counselling@charlottemedlicott.co.uk.

How I store your personal information

Your electronic information is securely stored on my computer using password protected files. Your paper-based documents are stored in a locked filing cabinet. I will keep your personal information and any process notes I make of our sessions for 6 years after you finish your counselling sessions with me. I will then dispose of your information by deleting your files from my computer.

Your data protection rights

Under data protection law, you have rights in relation to your Data:

a) Right to access: the right to request (i) copies of the information I have about you at any time or (ii) that I modify, update or delete such information. If I provide you with access to the information I hold about you, I will not charge you for this, unless your request is “manifestly unfounded or excessive.” Where I am legally permitted to do so I may refuse your request. If I refuse your request I will tell you the reasons why.

b) Right to correct: the right to have your Data rectified if it is inaccurate or incomplete.

c) Right to erase: the right to request that I delete or remove your Data from my systems.

d) Right to restrict my use of your Data: the right to “block” me from using your Data or limit the way in which I can use it.

e) Right to data portability: the right to request that I move, copy or transfer your Data to another organisation, or to you, in certain circumstances.

How to complain

If you are not satisfied with the way a complaint you make is relation to your Data is handled by me, you may refer your complaint to the Information Commissioner’s Office (ICO).
